BOSS GNU/Linux is an Indian Version of Linux developed at C-DAC, Chennai, India. BOSS is an acronym for Bharath Operating System Solutions. It provides Indian language support and additional features such as ?BossBulkDocument Converter, Boss Presentation Tool, SCIM, Plug-n-Play device support, automounting of all existing operating systems, BharateeyaaOO for OpenOffice with multiple Indian language support. BOSS is LSB 3.1 certified by FSG. BOSS uses d-i and the team found it really flexible enough to work with. BOSS Team will be proud to contribute to d-i and debian community as a whole. Skolelinux is a woody-based Debian derivative that was the first distribution to ship with the DebianInstaller. The installer images are not actually modified for Skolelinux, only a couple more udebs are added to the CD. Debian-Edu is the sarge-based update to skolelinux which also used the DebianInstaller. The source to the added udebs is in their svn repository at svn://svn.debian.org/debian-edu/trunk/src/debian-edu-install
Ubuntu uses a customised version of the DebianInstaller for their installation CD. Their LiveCd used to use DebianInstaller as part of its boot process, although nowadays uses an initramfs-based boot process instead (Debian's live-initramfs package was derived from this). Their changes to the installer are available via bzr; see this wiki page for details. Their live CD uses a custom graphical installer (Ubiquity) with d-i as a backend for many operations.
Freegeek made a customized d-i image called the Freekbox3,generated using Simple-CDD.
Takeshi Yaegashi has created a version of the installer for the PlayStation 3. There is a wiki with details, and a diff against standard d-i here. This will hopefully be merged back into stock d-i.
DPHYS3 (Debian Powered Hosts Yet Simple, Version 3) is how the Department of Physics at the ETH Zurich deploys their over 150 Debian workstations automatically via netboot using a preseeded d-i.
Lihuen GNU/Linux is an Argentinian Debian Lenny-based distribution developed at the LINTI by students of the National University of La Plata, focused on education and low-level users. It adapted d-i giving it a new graphic theme and using preseeding to avoid localization options and low-level matters. Then it used debian-cd to create the ISO images.
SprezzOS derives its s-i from d-i, modifying it as partially described here.
