Kenshi Muto maintains ISO images for Lenny, Etch and Sarge that use the newest 2.6.* kernel for i386 and amd64. http://kmuto.jp/debian/d-i/

Skolelinux is a woody-based Debian derivative that was the first distribution to ship with the DebianInstaller. The installer images are not actually modified for Skolelinux, only a couple more udebs are added to the CD. Debian-Edu is the sarge-based update to skolelinux which also used the DebianInstaller. The source to the added udebs is in their svn repository at svn://svn.debian.org/debian-edu/trunk/src/debian-edu-install
Ubuntu uses a customised version of the DebianInstaller for their installation CD. Their LiveCd uses DebianInstaller as part of its boot process (details?). Their changes to the installer are available from arch, see this mail for details. Apparently newer versions of Ubuntu will not use d-i as much as previously.
DCC Alliance uses d-i as the installer for their Debian-based distribution. It is not apparently changed much as of ?PR1 except the addition of a newer kernel.
Dick Visser has produced sarge ISO images that support the Areca SATA controller. http://www.tienhuis.nl/areca/
LliureX 5.09 uses d-i as their installer. It is modified to use a special Catalan locale and translations are added. Details
Sun Wah's distribution uses a graphical installer derived from d-i. These changes are in a subversion repository (where?) but have not been contributed back to the d-i developers.
Freegeek made a customized d-i image called the Freekbox3,generated using Simple-CDD.
Hiweed-Debian GNU/Linux is a Chinese distribution that apparently uses d-i's graphical installer.
Takeshi Yaegashi has created a version of the installer for the PlayStation 3. There is a wiki with details, and a diff against standard d-i here. This will hopefully be merged back into stock d-i.
Say GoodBye To Microsoft is a web site developed by Robert Millan that automates the process of downloading d-i and booting into it from windows.
DPHYS3 (Debian Powered Hosts Yet Simple, Version 3) is how the Department of Physics at the ETH Zurich deploys their over 150 Debian workstations automatically via netboot using a preseeded d-i.
Lihuen GNU/Linux is an argentinian Debian Lenny-based distribution developed at the LINTI by students of the National University of La Plata, focused on education and low-level users. It adapted d-i giving it a new graphic theme and using preseeding to avoid localization options and low-level matters. Then it used debian-cd to create the ISO images.
